Course Description

This 2-hour continuing education module provides registered nurses working in home health settings with an in-depth understanding of the Start of Care (SOC) process, the OASIS assessment tool, documentation standards, regulatory compliance, and how accurate SOC/OASIS documentation affects quality, reimbursement, and patient outcomes. It includes case studies, regulatory updates, best practices, and strategies to reduce errors in documentation.

Learner Objectives

By the end of this module, participants will be able to:

2. Explain the regulatory requirements for SOC assessments and OASIS-E/E1 (including recent updates) relevant to home health agencies.

3. Identify the key elements and data items required in the SOC OASIS assessment, including those that impact case mix, reimbursement, homebound status, medical necessity.

4. Discuss common documentation errors or delays in Start of Care and how to avoid them.

5. Apply best practices in completing the SOC assessment with accuracy, integrity, and timeliness.

6. Use case vignettes to practice SOC/OASIS documentation and SOC timing, recognizing documentation pitfalls.
